Welcome to Rejuve & Recover, LLC (“Company,” “we,” “us,” or “our”). By booking and receiving services from us, you (“Client,” “you,” or “your”) agree to the following terms and conditions. These terms and conditions constitute a legally binding agreement between you and Rejuve & Recover, LLC. Please read them carefully before using our services. If you do not agree to these terms, you must not use our services.
1. ELIGIBILITY FOR SERVICES
- YOU MUST BE AT LEAST 18 YEARS OLD TO RECEIVE OUR SERVICES.
- You must truthfully and accurately complete all required medical intake forms before receiving services. Providing false or misleading information may result in denial of service.
- A licensed medical provider (Medical doctor or advanced practice provider) will assess eligibility for IV infusion therapy based on your medical history and good faith exam (GFE). The medical director will review, sign, and approve the service before your IV infusion therapy administration. We reserve the right to refuse service at our sole discretion if we determine that IV therapy is not appropriate for you.
- Repeat Clients: The Good Faith Exam (GFE) is valid for a period of one year, unless there are significant changes in medical conditions that warrant an updated evaluation.
- New Clients: A Good Faith Exam (GFE) is required prior to receiving IV infusion treatment.
ANY CLIENT RECIEVING OUR IV HYDRATION INFUSIONS MUST HAVE BEEN CLEARED BY OUR LICENSED MEDICAL PROVIDER.
WE RESERVE THE RIGHT TO REFUSE SERVICE TO ANYONE FOR ANY REASON OR NO REASON AT ALL.
2. MEDICAL DISCLAIMER & RISK ACKNOWLEDGMENT
- OUR IV HYDRATION SERVICES ARE NOT A SUBSTITUTE FOR MEDICAL TREATMENT. We do not diagnose, treat, or cure any disease or medical condition.
- You acknowledge that IV hydration therapy involves potential risks and/or side effects, including but not limited to bruising, infection, allergic reactions, vein irritation, dizziness, nausea, and fluid overload.
- YOU AGREE TO DISCLOSE ANY ALLERGIES, MEDICAL CONDITIONS, MEDICATIONS, OR PREVIOUS REACTIONS TO IV THERAPY BEFORE RECEIVING SERVICES. FAILURE TO PROVIDE ACCURATE INFORMATION RELEASES US FROM LIABILITY.
- Our professionals may refuse treatment if they determine it is not safe for you.
- WE ARE NOT RESPONSIBLE FOR ANY ADVERSE REACTIONS RESULTING FROM UNDISCLOSED MEDICAL CONDITIONS, MEDICATIONS, AND/OR ALLERGIES.
- RESULTS FROM IV HYDRATION THERAPY VARY BY INDIVIDUAL, AND WE MAKE
- NO GUARANTEES REGARDING EFFECTIVENESS OR SPECIFIC OUTCOMES.
- You accept responsibility for any complications arising from undisclosed medical conditions, medications, allergies, or failure to follow pre- and post-treatment instructions.
- Medical disclaimer: The services provided have not been evaluated by the Food & Drug Administration. These products are not intended to diagnose, treat, cure or prevent any disease. The material on this website is provided for informational purposes only and is not medical advice. Always consult with your physician before beginning any therapy program. REJUVE & RECOVER, LLC DOES NOT PROMOTE EXCESS ALCOHOL CONSUMPTION.
3. BOOKING, PAYMENT & REFUND POLICY
- All appointments must be scheduled in advance and are subject to availability.
- Clients have the option to secure their appointment with a 30% deposit or pay the full amount at the time of booking. If you choose the 30% deposit, the remaining balance will be due prior to or on the day of your appointment.
- We accept the following payment types: Credit Card/Debit Card, Apple Pay, Google Pay, and/or cash.
PRICES ARE SUBJECT TO CHANGE WITHOUT PRIOR NOTICE.
Refunds & Cancellations:
- Cancellations made at least 24 hours before the appointment are eligible for a refund of the security deposit or rescheduling.
- Cancellations made less than 24 hours of the appointment will under no circumstances receive a refund from the security deposit made.
- NO-SHOWS WILL UNDER NO CIRCUMSTANCES RECEIVE A REFUND FROM THE SECURITY DEPOSIT MADE.
- Refunds will not be provided once services have been administered.
- CERTAIN FEES, INCLUDING CONSULTATION FEES AND TRAVEL FEES, ARE NON-REFUNDABLE AND NON-NEGOTIABLE UNDER ANY CIRCUMSTANCES.
4. PRIVACY, CONFIDENTIALITY & HIPAA COMPLIANCE
CONFIDENTIALITY & HIPAA COMPLIANCE: Rejuve & Recover, LLC is committed to maintaining the confidentiality of all client information in compliance with the Health Insurance Portability and Accountability Act (HIPAA). We adhere to all federal and state laws governing the protection of medical records and patient information. Any data collected during the booking process, medical intake, or treatment will be kept confidential in accordance with applicable privacy laws.
DATA STORAGE & SECURITY: Your personal and medical information will be securely stored in accordance with HIPAA regulations and only accessible to authorized personnel for treatment and operational purposes. We do not sell or share personal data with third parties except as required by law and is only accessible to authorized personnel for treatment and operational purposes.
CLIENT RIGHTS: You have the right to access, correct, or request the deletion of your personal data by contacting us directly. You also have the right to request a copy of your medical records and to be informed about how your data is used in accordance with HIPAA.
5. CLIENT RESPONSIBILITIES
Clients must follow all pre- and post-care instructions provided by Rejuve & Recover, LLC to ensure the best possible outcomes. Failure to adhere to these instructions may increase risks and impact results.
6. LIMITATION OF LIABILITY
ASSUMPTION OF RISK: By receiving services from Rejuve & Recover, LLC, you acknowledge and accept that such services carry inherent risks, including but not limited to personal injury, adverse reactions, complications, infections, allergic reactions, and fluid overload. You voluntarily assume all known and unknown risks related to IV therapy services.
RELEASE OF LIABILITY: You, on behalf of yourself and your heirs, assigns, personal representatives, and next of kin, hereby release, discharge, and hold harmless Rejuve & Recover, LLC and its successors, assigns, employees, contractors, officers, directors, agents, affiliates, and subsidiaries from any claims related to loss, damage, injury, complications, infections, medication reactions, or expense incurred due to the services provided, whether caused by negligence or otherwise. This release includes but is not limited to claims arising from errors in administration, unforeseen medical reactions, or failure to achieve desired results.
INDEMNIFICATION: You agree to indemnify, defend, and hold harmless Rejuve & Recover, LLC from any claims, damages, liabilities, costs, or expenses (including but not limited to reasonable attorney’s fees, court costs, and medical expenses) arising from your use of our services, failure to disclose pertinent medical history, non-compliance with pre- or post-treatment instructions, or any legal disputes related to the services provided.
NO GUARANTEE OF RESULTS: While we strive to provide the best outcomes, we do not guarantee any specific results from IV therapy services. Outcomes may vary based on individual health conditions.
GOVERNING LAW: This Limitation of Liability shall be governed by and construed in accordance with the laws of the Commonwealth of Virginia, without giving effect to any choice or conflict of law provision or rule.
SEVERABILITY: If any provision of this Limitation of Liability is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect to the maximum extent permitted by law.
NON-DISPARAGEMENT: Clients agree not to make false, misleading, or defamatory statements about Rejuve & Recover, LLC, its services, employees, or affiliates, whether online, in reviews, or through any form of communication. Any violation of this clause may result in legal action.
Under no circumstances shall Rejuve & Recover, LLC be liable for any indirect, incidental, special, punitive, or consequential damages arising out of or related to the services provided, including but not limited to lost profits, emotional distress, or financial loss due to medical complications.
7. MARKETING & TESTIMONIAL CONSENT
By utilizing our services, you acknowledge and agree to the following terms regarding the use of your image, likeness, and testimonial for marketing purposes:
- Confidentiality and Privacy
Rejuve & Recover respects your privacy. We will not disclose any personal or medical information in testimonials, reviews, or marketing materials without your explicit consent, as required by applicable privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA), if relevant. - Right to Modify or Terminate
Rejuve & Recover reserves the right to modify or terminate the use of any marketing or testimonial content at its discretion, without notice. You will be informed of any such changes that impact previously consented marketing materials. - Compliance with Laws
All marketing and testimonial practices will comply with applicable laws and regulations, including those governing advertising, consumer protection, and medical privacy, where applicable. You confirm that any testimonial or review you provide will be truthful and based on your own experience with our services.
8. NO GUARANTEE OF AVAILABILITY
All services are subject to availability, and booking an appointment does not guarantee service at a specific time or location. We reserve the right to reschedule, cancel an appointment, and/or provide a refund due to unforeseen circumstances and you will be notified in such cases.
9. CODE OF CONDUCT
Clients are expected to communicate respectfully with staff and follow all guidelines provided by Rejuve & Recover, LLC. Abusive, threatening, or inappropriate behavior will not be tolerated and may result in the termination of services without a refund.
10. REFUNDS FOR MEDICAL INELIGIBILITY
If a client is determined to be medically ineligible for IV therapy before treatment begins, they will be issued a full refund. This does not apply to cancellations due to personal preference or scheduling conflicts.
By using our services, you agree that Rejuve & Recover, LLC may use anonymized feedback, testimonials, or images (with your written consent) for marketing and promotional purposes. If you do not wish to participate, you may notify us in writing to opt out.
11. MODIFICATION OF TERMS
Rejuve & Recover, LLC reserves the right to modify or update these terms and conditions at any time without prior notice. Continued use of our services constitutes acceptance of any changes.
12. CONSENT TO TELEHEALTH
By booking an appointment, you acknowledge that a telehealth consultation may be conducted by an advanced practice provider to determine your eligibility for treatment. The medical doctor will be responsible for the final approval and signing off on the service before it is administered. You consent to receiving medical evaluations and recommendations through telehealth services.
13. TERMINATION OF SERVICES
RIGHT TO TERMINATE SERVICES: Rejuve & Recover, LLC reserves the right to refuse or terminate services to any client who violates company policies, exhibits inappropriate behavior, or engages in fraudulent or illegal activity.
NO REFUNDS FOR TERMINATED SERVICES: Clients who have their services terminated due to policy violations, abusive behavior, or non-compliance with medical recommendations will not be eligible for a refund.
14. NO MEDICAL INSURANCE COVERAGE
SELF-PAY SERVICE: Our IV hydration therapy and related services are not covered by medical insurance. Clients are responsible for all costs associated with their treatments.
NO CLAIMS TO INSURANCE: Rejuve & Recover, LLC does not process insurance claims, nor do we provide invoices for insurance reimbursement.
15. FORCE MAJEURE
Rejuve & Recover, LLC shall not be held liable for any failure or delay in performance due to causes beyond our control, including but not limited to natural disasters, government actions, labor strikes, pandemics, power failures, or other unforeseen events.
In the event of a force majeure situation, we reserve the right to cancel, reschedule appointments without liability, and/or issue a full refund of the ordered service.
16. COPYRIGHT & INTELLECTUAL PROPERTY
ALL CONTENT ON OUR WEBSITE, INCLUDING TEXT, GRAPHICS, LOGOS, IMAGES, AND SERVICE DESCRIPTIONS, IS THE PROPERTY OF REJUVE & RECOVER, LLC AND IS PROTECTED BY COPYRIGHT LAWS.
YOU MAY NOT COPY, DISTRIBUTE, REPRODUCE, OR USE ANY OF OUR INTELLECTUAL PROPERTY WITHOUT PRIOR WRITTEN PERMISSION.
